What type of benefit do you have?
The bank account owner must match the name of the policy owner.
The bank account must be active, under an accredited bank and in the same currency as the enrolled policy. Joint accounts cannot be enrolled.
Your mobile number must be active to receive the reference number for benefit pick-up.
The policy owner must present a valid government ID at any BPI or BPI Family Savings Bank branch upon pick-up.
If benefit is more than P500,000.00 or $10,000.00, choose credit to bank account instead.
The new or existing policy to be paid using the benefit should match the policy owner's name or their immediate family member.
If the benefit will only be partially used, you enroll another payout option to receive the remaining benefit amount.
If the benefit will be used as premium payment for an existing variable unit-linked policy, you must submit this form at least 28 days before the due date.
